The fat suit may be a running gag in the episode. In the promo where Leonard is in the fat suit, he's at Raj's and Raj has just taken a dish out of the oven. I got the impression that the implication is that Leonard is Raj's roommate, and Raj has been feeding him into obesity.

There could be another person's alternate reality where Howard is living with Raj - we see Raj pulling the same dish out of the oven and then we see Fat Howard - the result of Raj's cooking. Thus the running gag. They could possibly do a third one with Stuart.

It would definitely be in keeping with the existing gags about Raj's character.

From what I can tell from the promo pictures, anything is literally possible, and more than one character could have a fat suit on.
