Yeah, there was something in the arc reactor he made in IM1 that made him dying.

At least it didn't affect him now.. And I wonder why he needed surgery in IM3??

He chose to have the surgery to take away the remining splitter from the bomb in IM1. Which lead him to not need that arc reactor anymore.

It is not the same. He threw that away in the end of the third movie. And in Civil War he doesn't even have one. So sometimes between Civil War and Infinity War. He made an new arc reactor with the nanotec! (my guess he was trying to make a suit that will help him to be able to protect the earth better after the Avengers split up)
