According to the history - Cesare was her first man. I mean they spend her first wedding night together and then he kept visitiing her in her new home. SHe wasn't intimate with her husband at all. He was beating her BECAUSE he couldn't sleep with her. Imagine why!

So her first child is Cesare's.
