From what I remember it is from being tortured. They get him back & he dies in hospital in the US but its after complications from being held prisoner or he gets harmed in place of Callen or something. Without rewatching I couldn't say it but its towards the end of season 10. The episode after Anna escapes from prison, when Callen (& the team) go after her, they find she's actually looking for his father & has found him so they try rescue both of them
