That could be a big point, because if Elena was just reacting to his eyes or what his face does when he is.. turned on.. I just don't think she would scream..
She didn't when she saw something was happening with his eyes in the pilot.

I was also thinking... could be Stefan having a dream? Maybe he is scared of what could happen if he and Elena got physical with eachother, that he may lose control and he fears Elena's reaction...

It does make more sense for it to be Stefan. I don't think Elena has any real doubts in her head about Stefan being something out of the ordinary. Stefan has more of a fear of what would happen if Elena found out.

Yeah, that's very true. Probably the closer they become, the more he fears of her finding out.. and rejecting him. Especially with now Damon in town, those fears have come to a head.
