All tv shows shoot a pilot episode which they then send to the network. Most of the time they air the pilot as the first episode that's why the episode title is called Pilot. But in some cases like Heroes, they reshot the first episode because they weren't satisfied with the pilot or something. So Heroes 1x01 is titled as Genesis I think instead of Pilot.
