You can see how hard it is for her to tell him she needs him there. The way she looks a way because she is scared of what he will do. Buffy was used to all the other men just walking away. This girl has serious abandonment issues.
The look on Spike's face is of course awe. He just still can't believe that Buffy wants him in her life.
It reminds me of the look he has when she said she believed in him.
