Hee, we've come close in the GG game. There was one scene that no one ever managed to figure out within 20 questions. In any event, the guesses don't really advance the game the way the questions do, so it might be good to consider them a category unto themselves.
...And please let my last guess be the right one, hee. I honestly can't fathom what else fits all the criteria!
