I can't remember if Snape found out or if he knew about Scabbers being Peter...

I do think he probably knew because Peter disappeared when Sirius was sent to Azkaban. Then Remus found out that Peter was at Hogwarts after Harry mentioned seeing a Peter Pettigrew on the map.
