I guess in theory the crown would go to Kay? Because he's Arthur's right hand? Is that how things worked back then

Well, "back then" could refer to different periods, considering the Arthurian legends were started in one period but were later portrayed with the culture of a later period. Not to mention the show has zero historical authenticity. But I don't see Kay as being a candidate. No royal blood.
