We only see it at night I assume (and probably because it looks way cooler at night, LOL). Unless they had seen it during daytime in the books, which I've forgotten.

And it happened because Voldemort killed Cedric. UGH, I remember the movie part when Harry came back with Cedric and no one realized that Cedric is dead.

I can understand why. That spell was meant for him and not for Cedric. Cedric just happened to be there with him and was the one that got killed.
