I believe that in S5, Brian was so far gone because of his many insecurities, he never once took the time out to appreciate the ones who meant everything to him. And yes, all it took was that bomb. Sometimes mental crises (no matter what they are... his was the reality of getting older) has such a terrible way gripping someone that they just completely lose his or herself. That's definitely what happened to Brian, because I cannot imagine s1-s4 Brian going nuts like that.
