Good question. Dumbledore becomes Headmaster after serving as Transfiguration teacher for several decades. He replaces Armando Dippet. I don't think it stated in the book how he became headmaster or if he was elected for the position.

Same. It's usually done through voting by a group of professors. Also it looks like (for Hogwarts) that appointment holds until the headmaster's death or if he steps down (I think).
