You mean when Dumbledore called for Harry because he didn't come forward at first? Yeah Dumbledore was surprised. The Triwizard Tournament normally has only 3 champions/participants so Harry shouldn't even be picked as Hogwarts already has a participant c/o Cedric.

Yes it's always 3. The first time it was 4 was in GOF. After Cedric died, the tournament was discontinued.

That's they the professors all told Dumbledore not to let Harry join because he's young (there's an age limit to join) and the rules state only 3 champions/competitors. Dumbledore argued that the Cup's decision is final and that they should follow it.
