No, I think now she is def back on set, she won't leave again.

The episode she will appear in november was already done.

Now she is shooting the rest of the season, like everyone else.

I'm hopeful she will be in more then one episode in november
cause shonda said this week they ripped apart episodes 9, 10, 11 and 12
and putted them back together. So they replaced some scenes and changed others.

Because of that I'm hopeful they will manage to insert more izzie's scenes,
since Katie is back on set.

If shonda ripped apart episode 12, that means from now on they are shooting 13, 14, ....

so that means she won't leave again, cause her break was from 6x05 to 6x12,
appearing on 6x09.
