I think she just meant that it was still considered "cool" or "trendy" to smoke in some places. I just don't get it.

It's so disappointing that Spencer and Toby still smoke. I think Adelaide might have smoked at some point, too. I hope they all quit.

Yeah, it's hard to believe the smoking could still be considered "fashionable." A cigarette is not an accessory.
