I think where Fitz messed up in S1 mainly is that he didn't tell Simmons how he felt in different circumstances. He wasn't wrong to tell her, but he waited too long and then had to tell her then because he might not have another chance. It was beautiful that he did, it was that last scene with the two of them in S1 (and the consequences of that) that got me emotionally involved in this series.

But the way he did it, it was this huge, dramatic gesture that blindsided Simmons and she never got a chance to respond before he almost died. So now that confession is also wrapped up with coma and brain damage and recovery, where Simmons wants to help him and probably feels guilty because he's in this state now because he saved her life. At least at the beginning, she probably felt that telling him how she felt about their relationship would just hurt him more at a time when he already had a lot to deal with.

So I'm sympathetic to Simmons position, but I think her decision to leave without talking to him first made things worse - it might have been best for his recovery, but not their relationship. I agree with Bobbi that now Simmons needs to be more open and honest with him if she wants to repair that relationship - she may not tell him what he wants to hear, but just not talking about it isn't going to fix anything.
