The promo shows him basically fading away with Braeden calling his name like he's dying. It's just what I took from that 2nd half of S4 promo.

I think it was one of the things that made him so strong, his memories and experiences.

You're right, it looks like Derek died. If he lose the ability of healing himself he will be in danger.
