Well, Belle got to the Underworld while still alive, and Charming, Snow, Emma, Regina, Robin and Rumple travelled there, so it's clearly possible to get there without being dead. I guess we have to wait and see how and why Red is there.

The why is a good question. In the promo she was in Oz with Dorothy and Mulan so it has probably something to do with Zelena. Do you think Dorothy and Mulan are in the underworld too.
