I think it depends on the level of their fame whether they have an assistant or not. A lot of the guests I've met at cons haven't had assistants with them, they usually just bring their husband/wife/boyfriend/best friend/whatever. Occasionally the big stars have assistants though.

Getting to be the guest's personal assistant for the whole weekend is the best volunteer job at a con, though!
