It all depends on your layout. You can search for communities where people post layouts they've designed and it gives instructions. I got mine from babydoll19 but there's also a community called free_layouts. For mine, it gives Custom CSS code which you copy and paste into Custom Options and there's usually a bit at the end where you paste the url for your banner in.

Hope that helped a bit, I'm not the biggest LJ expert
