I think the rose stabbing one is a foiler.

and that is definitely a dream sequence.

I think Meredith might dream that Derek is in an accident, but the heart attack comes at a latter time in the ep? Perhaps as a cliffhanger of the premiere?!?!

Denny is when Derek wasn't in concious and sorta like Meredith drowned and was visited by Denny.

He is coming back. I think they may not have listed him, so he'll be a surprise?

They didn't show him in the promos either.
