That's what I thought of first because the blood looks like it's around his neck, and then was like.. oh no, he cut Sylar's throat, not the other way around seeing as they shot the dream sequence during the Untold Stories stuff, maybe it was during then and something happened to HRG that we never saw in the stuff we got.
