I think it's like how American ratings have NC-17 and X, though X isn't as common except for the dodgy ones!

I think X was phased out, actually, though you can still find X-rated movies if they're old. (I don't know how I know this stuff, I'm not even American!!)

In fact, according to the BBFC website we don't have a 21 rating any more, just an R-18 which is "To be shown only in specially licensed cinemas, or supplied only in licensed sex shops, and to adults of not less than 18 years."
