That's where it's hard to predict, because Cas seems dead before Sam or Dean. Because both of them are left standing! So maybe he dies protecting Dean, and Dean dies protecting Sam? That's what I'm wondering. Jensen did say he had a very hard time digesting the ending, but after seeing multiple points of views, he was better about it. So makes sense he'd be upset that Dean dies, when he's always envisioned Sam dying and Dean being left alone to wrestle his demons.
