Juan only started to look concerned about his rash actions towards Paolo when he saw the cold look on Lucrezia'a face. I think he desperately wanted Lucrezia to feel as protected by him as by Cesare, but the difference is that Juan was incapable of expressing himself as tenderly as his brother, therefore it all backfired on him.
