Short answer: A lot.
Longer answer: Basically, the show would become a drama with some funny parts if it took place modern times. The sometimes problematic punchlines of T7S wouldn't be acceptable. Laurie being shamed for her enjoyment of sex, for instance, would have to be taken seriously, debated, and ultimately rejected. The difference among the characters' economic backgrounds would become more apparent with today's technology. Pot is more potent now (for several reasons), and its legality is growing across the country. The list of what would have to change is in the dozens.
