They don't end up together. None of them (Nate, Serena & Blair) ended up with each other. Nate picked neither of them. He sailed off to explore the world and left both girls a message that he loves them both - always have always will - and that they should love each other. But I still think if Cecily were to make a book for the sole purpose of closure - he's definitely pick Serena.The prequel screamed volumes to me on why.
