well it looks like the eye technician screwed up and ONLY faxed the Even pages and didn't bother looking at the other side of the page to fax the Odd Pages too... so the Application a was rejected because of her....it was a Horrible mistake to have the Eye Clinic sign and fax my buss pass application
. Hopefully my regular Dr. office does a better job and faxes it correctly
